Mini Membership Sign Up

• I understand that my Membership is non-refundable and non-transferable.
• I understand that my Membership is no longer valid after its expiration date.
• I understand that in order to cancel my Auto-Pay Membership, I need to send an email
  to info@dev.myyoga.nl 1 week before my scheduled pay date and that all other forms of
  cancellation will not be accepted.
• I understand that this membership price is €65 per 4 weeks (28 day).
• I understand that I can use up the 4 lessons at any frequency I want during the 28 day
  period.
• I understand that any used classes will not be carried forward.
• I understand that I am allowed to suspend my Membership 1 time per year for a maximum of 8
  weeks and shall do so in writing to info@dev.myyoga.nl 1 week prior to the requested suspension
  time.
• I understand this agreement is a recurring payment every 28 days until cancelled in writing
  as mentioned below.
• I understand that MyYoga has the right to increase the price of the contract by the CBS
  index 1 x per year in the month of January.
• I understand that reservations for class must be made online in advance of attending a
  class.
• I understand the late cancel policy; whereby if I cancel my class less than 4 hours before
  the class or do not show up in time (5 minutes BEFORE class starts) I lose the lesson as if
  I had attended the class.
• I understand that if my recurring payment is rejected by my bank for insufficient funds or
  making a refund booking without prior written consent from MyYoga to do so I will be charged
  a processing fee of €20.
• I understand that my recurring payments will be deducted from the bank account (or credit
  card) for which I have given details. This payment will come from a “payment processor”
  which is not MyYoga but called Stripe Technology Europe Ltd. The name of MyYoga will be
  mentioned on the description of the payment.

Monthly AutoPay Membership Agreement
Description of Membership: 

• This is a mini membership allowing you full use of our facilities with 4 classes every
  billed every 4 weeks by automatic payment.
• Each lesson allows full use of our facilities and access up to any regularly scheduled
  classes at MyYoga.
• This membership shall commence on the selected starting date and shall continue on a 4 week
  basis (28 days).

Frequency of Payments will be automatically charged every 28 days to the credit card or
bank account (IBAN) provided below on the day the agreement commences. Each 4 weeks period is billed
in advance. No prior notice will be given. You will receive an email informing you when the charge
has been made.
Automatic Recurring Payment Failure: If your automatic recurring payment
is declined, for insufficient funds or by making a refund booking without prior written consent from
MyYoga to do so you will be charged a €20 processing fee.
Cancellation of Automatic Recurring Payment: You may cancel your membership at any time with a written notice by email info@dev.myyoga.nl at least
1 week prior to the next payment date. NOTE: Making a refund booking via your bank does not
constitute cancelling your membership. Continuation of Automatic Recurring Payment: The monthly
payments will continue for as long as you stay on the membership. Only by following the cancellation
rules will it cease.
Changing Automatic Recurring Payment Method If you would like to
change your Automatic Recurring Payment, including, for example, making a change from one
credit/debit card that was initially selected to another credit/debit card, you must authorize the
recurring payment with the new credit/debit card by notifying MyYoga in writing. Your new
information will be implemented before your next billing cycle for your membership.

AGREEING TO THESE CONDITIONS By providing my credit card / bank details (IBAN)
information (“Payment Method”), I AGREE that I have read and understand this Automatic Recurring
Payment Agreement. In addition, I authorize MyYoga to charge €65 once every 4 weeks (28 days)
to the specified Payment Method; and I authorize the financial institution for the Payment Method,
specified below to charge my account and remit payment for membership to MyYoga. This authority will
remain in effect until I give written notification, as required under this Agreement to terminate
this authorization. I agree to the general terms and conditions as stated on the
website www.myyoga.nl. Note: credit card / bank information is stored in a secure format on
MindbodyOnline, whereby employees of MyYoga do not have full access to all numbers and details of
your credit card / bank and therefor cannot authorize payment for anything further than what you have
agreed to here.
